Not sure what kind of extender you are talking about but I assume that you mean the vac-extender and the static stretcher?
Well, I started out with the Jes-extender and have since then tried both the vac-extender and the static stretcher.
The Jes-extender is not even a option since it is too bulky, too painful to wear for multiple hours (never managed to wear it for more then 1,5-2 hours at a time before I had to take it off for a break), often needs serious modifications (wrapping both the penis head and the base of the extender) and was invented by a charlatan that has made far to much money on it already + it is seriously over priced.
The static stretcher might be a good alternative if you are cut. I am uncut and had serious problems with the wrapping. It is not a device that you get, open, take out, view the DVD, put on right away and can wear for multiple hours. It has a high learning curve for both cut and uncut and I do not recommend it for uncut people.
The vac-extender is the best PE-device I have (and I have MANY PE-devices..). I only have the old model right now and only have two bad things to say about it; I seem to develop a sore on the ridge of my glans from friction between my glans and the constriction sleeve, the base will give you a scrotum burn like most extender bases.
I solved the scrotum burn by putting a baby sock over my scrotum and got in about 10 days of wearing it for 12 hours every day before the sore came. Since then I have had to take breaks, trying to let it heal enough for me to use it again.
The vac-extender has now been upgraded and I will receive the vac-extender 2 in a matter of days. It also features a new constriction sleeve so I hope I do not get friction sores from this one as well. The base of it is also cushioned with silicone to avoid scrotum burn. I will give it a fair review once I get to try it.
Also, I should mention that I am biased since I will be re-selling the vac-extender 2 on my future web-site. The above is my honest opinion none the less and I have some serious experience with all three extenders (except for the static stretcher that I could only bare to test for 14 days because of all the pain and hassle with the wrap... foreskin can sometimes be a hassle too..

Yes, remek, now maybe you understand why I am a bit more skeptic when it comes to the gains some people claim to have obtained? My gains are 100% real and I have picture proof of them as well as video proof of my last 1,2cm BPEL gain (from the IPR hanging way back).
My current routine is to wear the vac-extender for 12 hours 5-6 days per week, manual stretches every time I go to the bathroom (I use my vacu-hanger and a string tied to the door knob, using my own body weight to get a good stretch, SO/SU, L, R, SO/SU for 30sec each followed by 40 mild dry-jelq strokes. Saturday and Sunday I pump and jelq. Total rest days come when/if I feel I need them.
