remek, I'm kind of confused myself. The way I understand it (I may be wrong) smooth muscle is classified as peripheral blood vessels, meaning it dilates. This would make sense. As I mentioned earlier, caffeine is found in many headache medicines. Constriction of blood vessels is a known cause of many common headache types. It would makes sense to get relief the vessels would need to be relaxed/dilated.
The other type mentioned is CNS (central nervous system) blood vessels. These appear to constrict. This would explain why caffeine makes some people nervous and hyper-active.
If this is the correct understanding, it could mean caffeine would play a positive role in PE. Either way, it deserves further investigation.
