Being that he is "capable" of a really high EQ if the stimulus is there might eliminate a physical cause. His problem seems to have more of a psychological root. I don't think it's a smooth muscle volume problem. You would think that if his smooth muscle wasn't developed enough it wouldn't matter what he did, he would still only have retrograde erections. Being that his EL is longer than his FSL, he seems to have enough SM volume to push the tunica way out. It seems his tunica has trouble relaxing when he stretches.

I think I'm going to have to agree with your analysis. If his SM is developed enough volume-wise but is just weak, it may take much more stimulus and effort than should be necessary to achieve a maximum EQ. I think your recommendation of a smooth muscle focused routine will do him good.
