I'll try to answer these as best as possible. When doing this stretch you should feel the pressure in the perineum. I don't feel anything in my balls, maybe Dust can describe what he feels.
1) Very good question. The OK grip for me is palm up. This is what allows me to push upwards, causing the intense feeling in the perineum.
2) One hand should be used to grasp your penis and your balls in an OK grip. When you do this, you should have them both grasped at the base.
Let me try to go into a little more detail on this grip. You make an OK Grip. You take your index finger and your thumb and incircle your penis and your balls, at the "extreme base." Now, the palm of your hand should be on the perineum. To be more specific, the part of your palm that is right below the pinky finger should be where you push into the perineum (also in an upward motion so that you are pulling your penis and balls straight up.)
Now, your second hand is used to give an extra stretch. You use your second hand to actually pull the penis straight out (so the penis is being stretch in two spots. The first s pot, right at the base, and the second spot, whereever you can get a good grip and pull straight up.)
I don't know how this will work for you since your uncut. It shouldn't make a difference. But if you find a modification, feel free to post it here.
3) I feel this pressure below the testicles -- in the perineum (the area right between the anus and the testicles). The perineum is where the inner penis is. Hence the name, internal stretching.
