It seems that smooth muscle is most always the culprit when it comes to erection problems. The smooth muscle is the dynamic component in erections. It is soft, and in comparison with the tunica, much more easily damaged. There's probably not much, apart from a tear that could affect the tunica. I think being that the tunica absorbs most of the stress from hanging, this makes sense. Connective tissue provides support and structure. Not much more. Probably extreme hanging, in both weight and time, could cause erection problems, but girth work can easily irritate the SM to the point of hindering an erection.
