I'm about to start this 90 day routine but the stretching part confuses me. How long do I hold these stretches? And what does it mean when saying do 10 stretches in a big circle?

Regarding stretches duration, you may start by holding the stretch for 25-30 seconds per hold.
As for the circle stretches, you basically, stretch your penis in a circular motion like the clock arms for 10 times.A real man never hurts a woman. And do I just alternate stretching left then straight out then to the right?
Also before i saw this routine, for the past few weeks I've been doing stretches where I've been holding straight out, down, up, left, and right for 5 minutes each way. This is probably to much for a beginner right?

Yes, you hold the stretch for 25-30 seconds per direction, (left, right, up, down and straight out, in any order you prefer).
5 minutes per hold is too much for a beginner, and please don't forget to warm up.
Depending on how long you hold your stretch, I don't recommend the total time exceeds 10 minutes, preferably less, of course.A real man never hurts a woman. I hold for the time it takes to do 25 quick kegels, so around 25 - 30 seconds, as B8 said.
