Seeking opinions (weak erections, low libido for 20 years; ~550 test, 60 free)

Long story short:

Injured dong one night 20+ years ago (tried to hide erection with ex-GF, bending in down awkwardly while lying on couch). Never the same after that. Boners felt and still feel weak. Maybe 50-80% hardness.

Libido also declined. I get aroused about once a week max. Then, my erections are merely ok. About 75%, but premature ejac. My woman puts up with me. No morning wood ever, except for a month after major abdominal surgery for unrelated problem. I asked doc who said it was parasympathetic system resetting.

Test is ~550, and free is ~60. Had intervening varicocele, fixed with embolism. Another urologist diagnosed chronic epididymitis after I complained about burning in lower ab/groin.

Any thoughts about what treatment/tests to seek? 45 y/o. Otherwise healthy.

Have tried ED drugs, which kind of worked.

Sometimes the pelvic floor muscles get spasmed and clamp the blood/nerve supply to the penis .

A pelvic floor physio could diagnose this if you have access to one.
