My background is that I was an avid cyclist for years, had occasional constipation, and slouched at the computer. I'm currently 21.
About a year and a half ago I was vigorously masturbating at my computer and during the whole process I pressed down onto my erect penis, and for a second felt like it went inside itself, into the body. Following that I had intense generalized deep groin pain, lasting 3-4 days until gradually going away. I had burning while peeing. I thought that I damaged the suspensory ligament as the pain felt like it was at the base. However, the fact that the general pain went away did not stop the pain from lingering. It stayed on the right side near the base of the penis where my ischiocavernosus muscle runs.
After the incident my dorsal vein would bulge out and look inflamed while standing, my penis would look shriveled, and all the other veins would bulge out as well - especially the ones running down the side. It would be very difficult to get an erection while standing. While lying down, however, my penis would return to normal and fill up. And when I would get an erection, my penis would sink toward the right, and rotate counterclockwise while flaccid even more. It's still like that to this day.
My pain would be chronic, eventually being on the right side at the base of my penis where it joins with the pelvic bone, but not as far as the transverse perineal muscles. It would be a kind of electrical burning sensation that would worsen with sitting.
After several months of waiting for a urologist, he told me that there's nothing wrong and tried telling me that it's psychological. Which it obviously wasn't.
Several months following that, I went to a pelvic floor physiotherapist for several appointments and she told me that I have pudendal neuralgia. She did not check my penis but instead palpated my muscles and did myofascial release through my anus. She said that my pelvic floor is really tight, especially my coccygeus muscle. She also said that I have an anterior pelvic tilt on one side: my right side. To add on, she said that my hamstring is really tight on my right side. I noticed that my back curve is way more pronounced on my right side, which is probably why I've had low back pain only on my right side for many months prior. Particularly in the right spinalis thoracis from what it looks like. And on my right side I had a tight piriformis.
For several weeks I did the exercises she told me to do, including piriformis stretches, and loosened my pelvic floor. However, even this did not stop the pain. I ran out of money for the appointments and I'm trying to manage it on my own now.
Something else I've noticed is that the entire area underneath my penis bulges outwards on the right side, and my ischiocavernosus muscle gets stuck in between this bulging area and my pelvic bone. It's as if the base of my penis and the surrounding tissue deviated toward my right. I wonder if this is the cause of my pudendal neuralgia and erectile issues, since the IC muscle carries the pudendal nerve.
I've also noticed that my right ischial tuberosity is not nearly as mobile as my left ischial tuberosity when rotating my pelvis from a posterior to anterior position, and that there's a muscle (likely my semitendinosus muscle) that travels to the right of the tuberosity when in anterior position while it goes to the left when in posterior position.
When I started sleeping upright by putting a pillow underneath my knees just recently, my penis would start filling up, and I would even sometimes experience morning wood again. It would also alleviate the pain. But it only happens in this position as it fixes my anterior pelvic tilt. Could this be a lumbosacral issue due to the misalignment of the pelvis, because only one side is tilted? Could it be a lumbosacral compression from L5-S1 that's causing sexual dysfunction? I'm trying to search for answers.
I'm going to do further exercises to correct the anterior pelvic tilt, but I don't know which set of exercises to do to address it on one side. I would like some suggestions.
Is it possible that I could have somehow torn or damaged my IC muscle? Or even displaced my sacrotuberous ligament? I have a very girthy penis - 6 inches in girth - so I was wondering if the sheer weight of it combined with pushing it down caused it damage.
Why is everything messed up on the right side of my pelvis? Including the hamstring tightness, low back pain, bulging ischiocavernosus muscle, and mobile semitendinosus muscle.
I'm going to do a CT scan of my pelvis when I make enough money, but for now I'd like some feedback.
