Are there any common(or uncommon) injuries or risks associated with using an extender? How long is it safe to use one at a time, how much tension should I feel and what are the signals that I should stop or rest?

If your members no conditioned, start off slow with low tension then slowly work up, if you don't feel pain or major discomfort then you're ok, also if your glans turn dark purple or numb then remove and let the blood flow of course. Some purple and cold glans is normal and can't really be avoided but dont leave it on too long.If your extender has 3 tension lines I'd start at the first line for a month or so and start maybe 2-3 hours a day then slowly work up to as much as you can stand over the next few months.
Yeah seems from the studys the guys go at almost max tension like 8-12 hours a day, that seems crazy.

Am I supposed to feel much tension? Is the extender too short if I don`t?

Well does your extender have the 3 tension lines? If you unscrew the thumb screws it should move to atleast the first line, if it doesn't then you need to add bars to the extender

TJT, beware of possible nerve damage.
Google "extender nerve damage" and see what I'm talking about.
Personally, I can't imagine myself using a noose/strap extender-it is way too risky.
I'm trying to gather as much info as possible regarding the Vac Extender, which seem safer but is also notorious for blistering and slow gains.Stats:
5.25" BPEL
4.75" NBPEL (T______T)
4.75" MSEG

Noose isn't risky, and strap is even less. If you over do it and not watch for signs then it's risky, but only dumb people that try and go for 10 hours a day are asking for trouble, it's like someone jelqing hard for fast gains, you're going to get a serious injury. And the ones that did get nerve damage used the nooses without any padding, and most likely didn't watch for any signs of numbness etc.

PM Dangler. IMO, he is one of the extender gurus on this forum. He also has several reference links in his signature.
I personally use an X4 with a VacExtender Mod Kit. This set up has the advantage of allowing you to switch back and fourth between strap-in and vacuum cap. Big disadvantage is $$$. I love the VacExtender for its comfort. However, if vacuum cap is not seated properly, it can indeed cause bruising and blisters if worn too long when not properly seated. I usually do several 1.5 hour sessions which have never resulted in any problems, but this may be due to the fact I have been using it for about six months now and my glans is conditioned. I also use the cap and sleeve as an ADS with 20 oz of fishing weights attached. I average 7 - 10 hour sessions.Valued Member of 11+ years at the PEGym12/'09 (start) NBP EL - 4.5, EG - 4.4

Going a little off topic here-
Not2Big, have you seen any measurable gains using the vac-mod?
I've heard gains with it come slower than with a strap/noose.Stats:

If you're using an extender that uses tension, then the vac-mod will give you the exact same gains, because you will have the exact same grams of traction on your penis.
Even same with the non spring loaded tension extenders, if you set up the vac-mod to be the same length as using a noose/strap then you will get the same gains.
