If you are starting with an extender, your penis should be stretched about 1/2 inch past your erect length and in an upwards position.

You should increase the rod lengths as your penis grows. Once you set a new length setting. start with the extender in the upwards position. In time, you will move it to the middle position, then down. As you grow, you repeat the cycle over again.
