Today the tip of my penis was sore, and I stepped back and asked myself if I'm being too aggressive. I'm making great gains and would not want to mess them up. I think this is only because I pumped up in the bathmate to a new record, and I was not ready for this. For PE veterans, do you have any insight on how to make progress without getting injured? Can you think of ways people often do get injured?

I really do not know anything about devices. I wouldn't recognize a bathmate if it walked up and said "Hi!" at a party.
However, I do know there are ways to injure yourself when going for maximum expansion. I have used the "Uli Stretch", which is essentially using an ok grip to clamp the erection to make it expand. If you do it at a high erection level and are not careful it can lead to burst blood vessels. I have had that happen twice (in 5 years). It is not debilitating, but it it is scary and prevents PE while it heals.
The key is to always be mindful of what you are doing. Focus on your penis/erection at all times during PE work. It is tempting to try and do other things, or to incorporate porn. I say don't. Focus. Be aware of how it feels at all times. There is a distinct difference between a good stretch and a painful one. If it is hurting, stop.
Think about stretching a leg muscle. When you are doing it properly, there is a felling of a pull. Almost painful, but in a good way. But if you pull too hard, it will really hurt. That is not good. The same goes for your penis.
Always better to be safe than sorry.
Do warm ups like religion.
Be careful how you grip and pull.
Be mindful of how it feels.03/2015 Start <Able to last ~ 2 mins PIV>
