I had to take a 3 month recovery rest due to an injury and work related travel. I put my extender on after a 3 month rest and could strap in without shortening the bars. I do think I might have lost some minimal girth. IMHO, I think extender gains are permanent; or at least resistant to prolonged rest periods.

I think there is a lack of information on this subject as there aren't that many people that have reached their goals and aren't trying for more. From what I've seen, if you stop PE completely, regardless of how long you've been doing it, you stand to lose all or some of your gains. Might take years and you might retain some of your extra size, but I don't think I've seen anyone that has stopped PEing for more than a couple of years and has retained all their gains.
Some kind of maintenance is a smart idea.

This is something that i'm very interested in too. To my knowledge:
If extender gains really come from new cells being formed and thus over time making the penis longer (as the manufacturers claim), then it would make sense for the gains to be permanent.
But how much of the gains really come from new cells being formed i don't know.

A light maintenance routine and plenty of erections should make your gains permanent, but if you stop exercising completely then yes you will likely lose everything that you worked hard for.Starting stats : BPEL 6.25" NBPEL 5.6" EG 4.8"
Current stats : BPEL 6.8" NBPEL 6.25" EG 5"
Main Goal : BPEL 7.5" NBPEL 6.75" EG 5.5"
