Both happening is likely the case, but proving the actual cell growth is the tough part since it's probably still going to be some time before smooth muscle is completely understood and is known how it can grow without hormones from fetal development and puberty.
The most plausible thing that can be known about growth, which isn't actual growth per tissue sense, is that it's known from practices like Yoga that ligaments can be worked and over time become more elastic and can stretch much more than they usually would. Since the suspensory ligaments and tunica are basically the same type of ligament found in the rest of the body, stretching and jelqing are probably doing the same thing Yoga does. That, as well as better blood flow meaning more tissue is filled up, is probably the easiest to prove if anyone ever does a study on PE rather than poopooing the idea of penile enlargement without surgery.

A few things to look into in reference to soft tissue growth and expansion reference material: mechanical creep, biological creep, and "Davis' Law". Well, penis is a muscle but really not...so I guess it has the same cellar structure for deforming it self. There cant be any other explanation. Other than that, i've heard it might be water that is filling up... that does not sound good. can anyone tell me its not waterBPEL...7.0 ...7.75....8
