Intense manual exercises. As intense as your penis can handle and more as it can handle more. It takes a great deal of intuition as to how much your penis can handle, but if I want to maximize gains, this is what I'll do.
Those gains are intense. I estimate I gained just under an inch in BPEL and over a quarter inch in MEG in 6 months.
Of course rest is neccesary

I gained half an inch in 6 months. I started at 50 jelq squeezes, 50 circle streches, 50 kegels and one 30 second Uli. Every other week I would add 10 squeezes, 10 streches, 10 kegels and add another uli. I warmed up before and after each workout and took weekends off. Eventually I stopped at 250 jelq squeezes, 100 circle stretches, 100 kegels and ten 30 second Uli's. On off days I would edge once in a while. Went from 5.5 x 4.2 to 6.6 x 4.8 (base girth). And that was with about 14 months of PE. ALSO I would sometimes do the exercises over 2 workouts. Do half of the workout early in the day, and the other half later in the day.

I gained about an inch in the first 3-4 months. I did a modified version of a JP90. I exercised M,W,F. Always warmed up and cooled down. I would take two weeks off instead of one at the end of a 10 week cycle. I always made safety a priority and progressively built up to more reps slowly. It has been cemented so I have a permanent gain of 1 inch. And about a half an inch in girth. I'm currently at about 7 X 5.5 trying for the magical 8 x 6(at the base). Sigh, one day perhaps. BPFS is just under 8".
