I believe this exercise is similar to a a compressor. Where you squeeze the base of your dick and below the glans and try to expand your mid shaft girth.
I don't think you should do the exercise fully erect or you may break your dick. Once your dick is hard it is not flexible.
I think doing them at 70 % erection or less is safer.
